Medical Waiver Deadline!!

Area Directors, please contact your teams!
Managers, the cut-off date for Medical Waivers is November 1st. Please contact Mike Knowles ASAP with your request. You will need a WRITTEN MEDICAL REQUEST from the Player's doctor, surgeon or some professional office. Call Mike at 941-725-0790 to start the process or with any questions.
Currently, Players must show their FHC card and photo ID at the check-in table. They will need both of these at the December tournament. Check with your Players now to make sure they have their card. Do not wait until December.
